Lease-to-own - $143/month for 12 months (includes survey tablet, docking station, software and one year warranty).  Add $58/month (for hosting & storage and telephone technical & installation support)

Outcomes Performance Reports
Monthly ($45), quarterly ($75) and annual ($125) trended aggregate reports to show performance compared to state and national norms can be purchased as desired. 

Return-On-Investment (ROI)
